Home & Community Based Service Options
Many older adults want to continue to live in their home as long as possible but may require help to remain in that home. Home and community based services that are available to purchase include, but are not limited to, home health care, home maker / chore services, respite care, adult day care, and hospice care.
Home Health Care
Home health care services are provided to older adults on an ongoing basis, or for short periods of time, and are intended to promote, maintain, or restore the health of older adults in their home. Registered nurses are available to monitor medications, help with wound care and other medical services. Therapists can help provide physical, speech, respiratory, and occupation therapies in the home. Home Health Aids are available to provide support services for personal care like bathing, dressing, grooming, meal preparation, light housekeeping, and running errands outside the home.
Home Maker / Chore Services
Home maker and chore services benefit older adults by providing assistance with non-medical services. Many of these services include things like chores around the house, cleaning, meal preparation, laundry, washing floors and windows, shoveling snow, mowing lawns and shopping. Services can be tailored to each person's needs and can be provided on a short term or ongoing basis.
Respite Care
Respite care gives the family and/or primary care giver a short term break from their daily responsobilities. Older adults receiving care in their home can be temporarily moved into a facility to receive the services and support they require. Alternatively, a home care agency can be hired to provide these needed services in the home without the need to transfer the person receiving care to a facility. Respite care is a temporary solution provided for one week to one month, depending on how much time the family or care giver needs.
Adult Day Care
Adult day care benefits older adults that need a variety of health, social, and related support services during the weekday but have the support they need at night. Seniors who attend an adult day program usually arrive in the morning and leave in the evening. Throughout the day there are scheduled activities each attendee is encouraged to participate in. Personal care and health related services are also available.
Hospice Care
Hospice care specializes in comforting measures and counseling to provide social, spiritual, and physical support to terminally ill patients and their family. Hospice care is a choice the patient makes to discontinue curative medical treatments and replace them with palliative (comfort-providing) care for their terminal illness. A physician-authorized document is required to qualify for the Medicare Hospice benefit. This type of care can be provided in a person's own home or in other care settings like assisted living and skilled nursing facilities.
